Choice Program Applications for the school year
​2024-2025

Open enrollment for the 2024-2025 school is still available for all families wishing to utilize the Milwaukee (MPCP) or Wisconsin (WPCP) Parental Choice Program. 

If you have questions, please feel free to call us at the school office 414-282-7970 (Leave a message with you name, grades of the potential student(s) and your phone number.  If we don't answer, we will call you back!) Office hours are
Mon-Fri, 7:30am to 3:45pm

*For current families: Submit you application along with your Proof of Residency - Click here for the list of Acceptable Documentation

*For New families: we will call you once you submit your application to set up an appointment time for you to bring the following documents:
  • Choice Paperwork (Proof of Residence and/or Proof of Income)
  • Birth Certificate (original document)
  • Immunization Records (latest available)
  • ​Sacramental Certificates (if any)
  • Any other important documents depending on the situation (we will let you know what we will need)
