For Current Student Athletes: Please submit the form below for your student athlete prior to their first practice. As a reminder, physical examinations are valid for 2 years. If your student had a physical 4/1/2022 or later, it is current for this school year. If your student had a physical 3/31/22 or earlier, s/he will need to get a new physical for this school year. Please schedule an appointment as soon as possible and get this form filled out.
